Authorization to Obtain Credit Report 

The undersigned hereby authorizes OnVirtu, its agents, and/or assigns to make inquiries into the banking, trade, and credit references provided, and to obtain credit and financial information from any credit bureau or other sources deemed necessary to establish creditworthiness. This authorization shall remain in effect as long as the applicant maintains an account with OnVirtu.

Personal Guarantee (optional, if required for small businesses)

In consideration of OnVirtu extending credit to the applicant, the undersigned individual(s), jointly and severally, unconditionally guarantee and promise to pay all obligations of the applicant to OnVirtu, whether now existing or hereafter incurred. This guarantee shall be a continuing guarantee and remain in full force until all obligations are satisfied in full. The guarantor(s) waive notice of default, nonpayment, and demand.

Payment Terms Agreement: 

Standard payment terms are Net 30 days from the date of invoice, unless otherwise agreed to in writing. Accounts not paid within terms are subject to a late charge of 1.5% per month (18% per annum) or the maximum rate permitted by law. Applicant agrees to pay all costs of collection, including reasonable attorney’s fees, incurred by OnVirtu in enforcing the terms of this agreement. Title to goods shall remain with OnVirtu until payment is received in full.
